项目简介

牙周炎是危害人类口腔健康的首要疾病。牙周炎造成的牙齿支持组织和结构进行性破坏是成人牙齿缺失、老掉牙的根本原因。牙周炎治疗的难点和关键是让失去的牙周组织获得再生,然而临床上缺乏行之有效的方法。16年来,在国科金等10余项课题支持下(累计经费超过500万元),本项目瞄准国际前沿,探索生长因子控释、生物材料仿生、干细胞移植促进临床牙周再生治疗的新方法,建立系列技术体系用于牙周病临床治疗,经国内56家足球比分直播,足球比分直播188:临床验证,疗效确切。

一、生长因子是促进牙周再生的重要工具,但直接使用极易失活,需反复使用,且易产生毒副作用。本项目成功研制具有不同药物释放效能的缓、控释系统,可以满足不同组织再生需求;在生物材料的多孔结构中,引入生长因子载体,实现药物载体与组织工程支架一体化;将不同控释载体与支架材料交联整合,实现两种因子有序、可控释放,从而建立双因子协同控释技术。J Control Release前任主编,2012年发表述评,详细介绍我们的工作,并再版双因子控释的结果。为进一步提升生长因子控释效果,课题组创新性地利用微球载体膜孔结构接枝温敏开关,实现生长因子智能化控释。以上研究结果,在J Control Release等重要期刊上,发表论文SCI论文16篇。利用课题组研制的生长因子系列载体,大大减少了生长因子临床应用的剂量和浓度,节约了成本,提高了治疗效果。

二、研制牙周仿生生物材料,是实现牙周再生的另一重要途径。为达到生物材料结构仿生,课题组将人体组织脱细胞处理后,获取细胞外基质信息,数字化并导入3D打印系统,建立精密仿生技术,体外成功复制出具有牙槽骨-牙周膜-牙骨质等复杂结构的支架材料;将归巢诱导因子载体,与支架材料交联整合,完成仿生生物材料细胞募集性设计,封面故事在Adv Biosyst上发表;随后,通过分子机制研究,回答了内源性干细胞体内迁移、募集的机理。以四篇专论形式,详细阐述了内源性再生策略、技术和方法,丰富和完善了牙周内源性再生理论。国外学者认为我们在牙周再生方面“迈出关键的一步”;并指出诱导机体干细胞归巢是一项具有广阔应用前景的临床再生技术。由于系列工作积累,受邀在材料学权威期刊Prog Polym Sci上,发表83页长篇综述,总结作为临床医生,课题组对“人体来源生物材料”的思考、探索和实践。此领域,共发表SCI论文15篇,其中5篇发表于材料学权威期刊Biomaterials杂志。动物实验和临床应用中,证实项目组研制的细胞募集性生物材料,具有激活机体内源性再生系统,募集内源性干细胞,修复牙周组织缺损的效能。

三、牙周干细胞治疗为牙周缺损修复和晚期牙周炎治疗带来新希望,但如何推动实验室获得的研究成果快速临床转化是必需解决的关键问题。课题组历时4年,完成世界首个干细胞治疗牙周炎随机对照临床试验,加速干细胞牙周再生治疗临床转化进程;围绕临床转化中的关键问题,如细胞来源、培养方法、移植设计和疗效评价等进行深入研究,共发表SCI论文17篇,并制定出一整套《牙周干细胞治疗》临床标准;被FDA临床试验中心同类研究采纳。世界牙科研究会主席高度评价课题组的贡献,对于整个再生医学都将产生影响。进一步项目组将依托国家口腔疾病临床医学研究中心,组织开展多中心、大规模临床试验。

通过以上研究,课题组建立了生长因子控释、仿生生物材料设计和干细胞移植系列牙周组织再生治疗技术体系,发明了3项新技术,完善了1个新理论,制定出1个新的临床标准。本项目共发表SCI论文48篇;总影响因子242;发表中文论58,授权国家授权专利5项;参编国内外教材4部。建立了结构合理、年富力强的牙周组织工程与再生团队,2017年获批“陕西省重点科技创新团队”。举办国家级继续教育学习班12期、省级学习班6期,在国内56家足球比分直播,足球比分直播188:,疗效确切,未发现不良反应,取得良好社会和经济效益。
